Description:                 This position is to welcome residents, provide psychosocial support, and monitor their comfort, concerns, and needs, assist in assessments on each resident on admissions, conducts and develops a social service care plans, yearly updates on social histories, advocate for resident’s rights, conduct a resident’s rights council meeting monthly, indirect and direct services for each resident, documents in progress notes on each resident, complies with State requirements, is a collaborative team member to all departments, assist residents and family on Medicare and Medicare D, Durable Power of Attorney, Do not resuscitate, assist in applying for Medicaid, provide social services to hospital and swing bed patients, discharge plans, ensure each resident has annual eye screens and dentist check ups or as necessary,  documentation on each resident, clerical and administrative duties, public speaking, marketing, and be familiar with Abuse, Neglect, and Exploitation reporting.

 

Essential Job Functions:         

 

Essential job functions are completed proper documentation required for the Social Services Department, assess new potential residents with Director of Nursing, clerical work, attending trainings and seminars, and QA monthly report.

 

Ensure quality care and documentation on the residents and patients. 85%

  • Directly report to your supervisor.
  • Welcomes new residents and visits them often to provide psychosocial support and monitor their comfort, concerns, and needs
  • Will be responsible to assess each resident’s needs at admission by obtaining input from the resident, resident’s representative and interested family members, which assist in developing the social services care plan.
  • Will assist in helping the resident adjust to the facility, exercise their rights as a resident, and continue their relationship with family and the community.
  • Promote facility-community interactions with residents.
  • Meet for 15-minute one-on-one visit with each resident per week for social services. It is encouraged to spend more then 15-minutes with residents.
  • Complete appropriate assessments, social histories, and MDS on each resident and will be responsible for writing progress notes on each resident’s response to the social services care plan when required by facility policies.
  • Provide advice and appropriate referrals to minimize social and economic obstacles to discharge, and coordinate discharge planning communications and documentation.
  • Assist in compiling information from individuals and/or families if necessary for hospital or LTC admissions (including but not limited to swing bed documentation, progress notes, and charting). Will also be responsible for clerical work, i.e., making copies, filing progress notes, mailing out letters, picking up the mail, phone calls, etc.
  • Will be responsible by communicating with resident and/or families of residents to ensure resident’s needs are being meet.
  • Will be responsible for facility a resident council once a month and a caregivers meeting on a quarterly basis; and provide documentation for both meetings.
  • Will attend funerals, memorials, and offer having a memorial for the resident at the LTC.
  • Will write the memorial letter, mail it, and send sympathy cards.
  • Will meet with a licensed social worker 4 hours a month and if asked will do additional paperwork as needed, i.e. signing off on social histories, etc.
  • Will ensure that the residents’ are receiving quality life care, i.e. self-determination, each resident’s needs are being individualized to meet that individual needs.
  • Will be responsible for each resident has annual updates on dental and eye exams and documented.
  • Must have good communication skills with patient’s and/ or residents and their families of various ages.
  • Will be able to develop good public relations, i.e. public speaking, making regular visits to the senior centers for a meal and provide them information and services that are offered at the LTC and hospital.
  • Assists with all of the above for hospital swing bed patients and senior living facility.
  • Must adhere to the confidentiality and HIPPA policy of Satanta District Hospital, LTC, and Senior Living Facility.
  • Must adhere to the State and Federal Regulations and review them yearly to keep updated on any changes and provide documentation.
  • Must be familiar with County, Community, and State resources that would enhance the services for the patients and/or residents.
  • Will keep updated on any changes for care plans and MDS.
  • Keep updated on swing bed paperwork and documentation as required to maintain proper documentation and services for the patient.
  • Complete proper paperwork and letters for memorials.
  • Will review yearly Abuse, Neglect, and Exploitation reporting and provide documentation.
  • Attends required CNA Meetings and annual orientation meetings.
  • Follow policy and procedure.
  • Maybe subject to other duties not listed.

 

Continuous Professional Development – 15%

  • Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.
  • Participates in hospital meetings and services organizations as required.
  • Participates on a hospital committee.
  • Attends monthly meeting with supervisor.
  • Attends monthly staff meetings.

Education and/or Experience:

 

Bachelor’s Degree:           Preferred

 

High School Diploma:       Required

 

Experience:                      Recommend at least 3 years in Social Services and working with the                         

                                         elderly population, and have good writing skills.

 

Certificates, Licenses,

Registrations:                   Required to have completed the Social Services Designee course and  

                                         maintain a Certificate
